Intensive Case Management/Community Support Team- services individuals diagnosed with services individuals diagnosed with a severe mental illness. The program aims to reduce the frequency of hospitalizations and targets those individuals possibly isolated from supportive services. A low staff to participant ratio allows for more individualized attention. Staff render services primarily in community settings and often in the participant's home. Services include Clinical assessment, Treatment planning; Psychiatric services; Medication Monitoring and training; Individual and group counseling; Peer support group; Intensive outreach; Advocacy, linkage, and external consultations with other providers; Active community integration through case management and social rehabilitation; and 24-hour emergency services.
